Lifeline

Lifeline - an emergency response system available to residents of Sagamore Hills
Township.

Sagamore Hills Township applied for and received a Community Development Block
Grant from Summit County to be used to provide a free Lifeline Program service to
eligible township residents.

What is Lifeline?

Lifeline is a 24 hour personal emergency monitoring system that allows you to call
for help with a small button that you wear around your neck or wrist.

When you push the button, Info Line enacts your personal emergency plan. Your
volunteer responders will come to your aid.

If you need professional help, Info Line can summon Sagamore Hills' EMS or police
to help you.

Lifeline helps residents remain in their homes when they:
• Live alone
• Are an older adult
• Are homebound
• Have medical problems
• Are recovering from an illness or injury

To get Lifeline, you must:
• Have a telephone
• Complete an application and be determined as eligible
• Have Info Line install a Lifeline unit in your home
• Have at least 2 volunteer responders who have access to your home and will help
you in an emergency

Click Here, for an application to print,
Complete the application and return it to:
Sagamore Hills Township
11551 Valley View Road
Sagamore Hills, Oh 44067
